In general, the Informed Consent Document must include signature lines for:
- the subject AND [See Example #1 and Exception #1]
- the person who obtained consent AND [See Exception #2]
- for studies involving children, a parent or legal guardian; for studies involving cognitively impaired individuals, a legally authorized representative.
